Important Note for Heirs and Executors:

In the unfortunate event that one of our songwriters should pass away, Cal IV Entertainment will direct all royalty accountings and payments according to the writer's last will and testament. In order to change direction of payment, the heirs and/or executors of the estate must contact us in writing informing us of the passing. Please be certain to include a notarized payment direction letter from the attorney handling the estate, a copy of the death certificate and a W-9 form showing tax information for whom future payments should be made. We acknowledge that this is a sensitive issue, and it is important to us that the families involved are treated with the utmost respect and sympathy. Please address any questions, comments or concerns to our royalties department.
